Last Updated: Tuesday, 20 August 2024Heart failure means that the heart is unable to pump blood around the body properly. It usually happens because the heart has become too weak or stiff. Heart failure does not mean your heart has stopped working but it does need some support to help it work better.

How we will help you manage your Heart Failure
To help you manage your Heart Failure, we will invite you to attend the surgery for an annual review. This is an opportunity to check that you are getting the best care you need and to stay healthy. It also gives you the opportunity to discuss any concerns that you might have regarding your health. You will be seen by one of our Practice Nurses who will support and advise you on how best to manage your Heart Failure. Having a healthy lifestyle - eating a balanced diet, doing exercise and not smoking - can help with your symptoms, which we can discuss with you. Our Nurses can also signpost you to services that may be of benefit to you in managing your Heart Failure.
